2012 Registration Form — Use this form to register for camp by mail or fax.

2012 Check-In Packet — CrossRoads must have a completed Release Waiver,  Health Form and a copy of your insurance card by opening day for the camper to attend camp. To qualify for Express Check-in the Release Waiver and Health Form must be  received with a copy of your insurance card 3 weeks before attendance at camp. Also included in the check-in packet is the camper packing list. This document contains everything you’ll need to bring to camp with you and a few items we’d like you to leave at home.

2012 LIT Packet — LIT applications and reference forms need to be received 3 weeks prior to your arrival at camp to be considered for the program.

2012 Volunteer Packet — Complete this packet and mail it to CrossRoads if you are an adult who would like to serve as a weeklong volunteer.

Bass Scholarship Form — Complete this packet and mail it to CrossRoads to apply for a summer camp scholarship. The guidelines for eligibility are included.

2012 New Staff Application — Complete this packet and mail it to CrossRoads if you have not worked at CrossRoads in the past and would like to apply for a summer camp staff position.

2012 Returning Staff Application — Complete this packet and mail it to CrossRoads if you have worked at CrossRoads in the past and would like to reapply for a summer camp staff position.

2012 International Missionary Information & Application — Complete this application if you are an international student or young person interested in serving for the summer at CrossRoads.
